Attachment Option in Create Service Request

Using the Create --> Service Request action in Locations application I want to be able to include an attachment, this option doesn't exist in the "Create SR" window that opens.

Using the Create --> Service Request action in Locations application I want to be able to include an attachment, this option doesn't exist in the "Create SR" window that opens. I've used application designer to add the attachment option to this window however when I actually add an attachment and save Maximo creates another two SR's. Anybody experienced this and/or have any idea on hoe I can resol
